Wickenburg Community Hospital’s Health Information Management department (HIM) is responsible for keeping complete medical records for each patient.
Out of an abundance of caution and to create a safe and secure environment for our patients and our health care workers during the COVID-19 outbreak, Medical records and billing services are available by appointment only:
The HIM office is located off-site, away from the hospital campus at 510 North Tegner (click for map).
Office Hours | Monday – Friday 8:00 am – 4:30 pm .
You can request copies of your medical record information by contacting a representative at 928-684-4364 or by faxing a request to 928-684-9406. Prior to receiving the copies, a standard “Release of Information Consent” form must be completed and proof of identification provided.
