In-Patient Rehabilitation

In-Patient Rehabilitation Swing Bed Program:

The  Swing Bed Program provides in-patient rehabilitation services for individuals with needs that extend beyond an “acute” hospital stay (72 hours). Swing Bed is 24 hour skilled nursing care plus the benefit of “rehabilitation” therapies to help patients transition to home or a long term care facility.  As a Swing Bed patient you will receive physical therapy, occupational therapy and speech therapy as indicated after surgery, illness or accident.

Swing Bed helps prepare you for the transition from hospital to home. It gives each patient time to heal and adjust before returning to everyday life. Our main purpose will be to provide you the necessary care through a collaborative team approach, facilitating therapists and skilled nursing to obtain the best possible outcome.

Who Can Benefit?

In many cases patients are not ready to return to their typical living environment following a hospital stay. That’s when our swing bed program can help! The needs of every individual are unique and we offer tailored rehabilitation plans for patients with medical history that includes but is not limited to:

  • Post-surgical care (cardiac, joint, etc.)
  • Exacerbation of chronic conditions
  • Injury from Motor Vehicle Accidents
  • Stroke or Brain Injury
  • Injuries Related To Falls
  • Deconditioning Due To Illness

Who Qualifies?

You can qualify for rehabilitation as a swing bed patient if you meet the following criteria:

  1. Have spent three or more midnights as “Acute Care Inpatient” at any hospital within a 30-day period.
  2. Were not hospitalized as an  “Observation Patient”

Who Pays For  “Swing Bed” Care?

Medicare part A generally pays for “Swing Bed” care, as do Medicaid and many commercial insurance policies. Our admission staff is here to verify your benefits.
