Sometimes in our lives there comes a need for a little extra help recovering from an illness. Transitional Care Programs can offer that support. Wickenburg Community Hospital has a Transitional Care Program.  You may be asking yourself, “What is Transitional Care, and how can it help?” The term transitional refers to transitioning from one level of care to another. If you have a three-day qualifying stay in any hospital within the last thirty days and require nursing or rehabilitation services to get you back to your best physical self, transitional care may be for you. 

Our experienced Hospital Physicians, caring and compassionate nursing staff, and our rehabilitation specialists are here for you.  We provide you with 24 nursing care, physical therapy, occupational therapy, and speech pathology services. In addition, our program provides an activities coordinator who will explore your interests and offer activities based on your input.  In the transitional care program, you work with your healthcare team to determine goals for your recovery. The program is individualized just for you.  Generally, the length of stay is less than two weeks, and while in the program you will receive physical and occupational therapy two times daily and speech services as needed and directed by your physician. 

Our program keeps you close to home near family and friends.  We know that being close to the ones we love and care about improve our ability to recover. At Wickenburg Community Hospital you will experience that hometown feel to medical care. You can expect to be treated with dignity and respect when it is time to leave the program your case manager and healthcare team will provide you with a discharge plan that meets your needs and will answer any questions you may have.
